TheserumamyloidA(SAA)familycomprisesanumberofdifferentiallyexpressedapolipoproteins,acute-phaseSAA1andSAA2,theformerbeingthemajorcomponentinplasma,andconstitutiveSAAs(C-SAAs).AlthoughtheliveristheprimarysiteofsynthesisofbothSAAtypesextrahepaticproductionhasbeenreported.Theinvivoconcentrationsincreasebyasmuchas1000-foldduringinflammation.Severalstudieshavestresseditsimportanceinthediagnosisandmonitoringofvariousdiseases.PathologicalSAAvaluesareoftendetectedinassociationwithnormalCRPconcentrations;SAArisesearlierandmoresharplythanCRP.
Recently,abroaderviewofSAAexpressionandfunctionhasbeenemerging.ExpressionstudiesshowproductionofSAAproteinsinhistologicallynormal,atherosclerotic,Alzheimer,inflammatory,andtumortissues.SAAhasbeenfoundtohavebindingsitesforhighdensitylipoproteins,calcium,laminin,andheparin/heparan-sulfate.Alsoadhesionmotifswereidentifiedandnewfunctions,affectingcelladhesion,migration,proliferationandaggregationdiscovered.ThesefindingsemphasizetheimportanceofSAAinvariousphysiologicalandpathologicalprocesses,includinginflammation,atherosclerosis,thrombosis,AA-amyloidosis,rheumatoidarthritis,andneoplasia.SAAhasalsoanumberofimmunomodulatoryroles,itcaninducechemotaxisandadhesionmoleculeexpression,hascytokine-likepropertiesandcanpromotetheupregulationofmetalloproteinases.Itenhancesthebindingofhigh-densitylipoproteintomacrophagesandthushelpsinthedeliveryoflipidstositesofinjuryforuseintissuerepair.Itisthusthoughttobeanintegralpartofthediseaseprocesses.Inaddition,recentexperimentssuggestthatSAAmayplaya"housekeeping"roleinnormalhumantissues.
ElevatedlevelsofSAAovertimepredisposetosecondaryamyloidosis,extracellularaccumulationofamyloidfibrils,derivedfromacirculatingprecursor,invarioustissueandorgans.Themostcommonformofamyloidosisoccurssecondarytochronicinflammatorydisease,particularlyrheumatoidarthritis.
