Themonoclonalantibody5G9recognizesComplementFactorC3b/iC3b,itspecificallybindshumanC3aswellasthebreakdownproductsC3bandiC3b.C3playsacentralroleintheactivationofthecomplementsystem.Itsactivationisrequiredforbothclassicalandalternativecomplementactivationpathways.PeoplewithC3deficiencyaresusceptIBLetobacterialinfection.OneformofC3-convertase,alsoknownasC4b2a,isformedbyaheterodimerofactivatedformsofC4andC2.ItcatalyzestheproteolyticcleavageofC3intoC3aandC3b,generatedduringactivationthroughtheclassicalpathwayaswellasthelectinpathway.C3aisananaphylotoxinandtheprecursorofsomecytokinessuchasASP,andC3bservesasanopsonizingagent.FactorIcancleaveC3bintoC3candC3d,thelatterofwhichplaysaroleinenhancingBcellresponses.Inthealternativecomplementpathway,C3iscleavedbyC3bBb,anotherformofC3-convertasecomposedofactivatedformsofC3(C3b)andfactorB(Bb).OnceC3isactivatedtoC3b,itexposesareactivethioesterthatallowsthepeptidetocovalentlyattachtoanysurfacethatcanprovideanucleophilesuchasaprimaryamineorahydroxylgroup.ActivatedC3cantheninteractwithfactorB.FactorBisthenactivatedbyfactorD,toformBb.Theresultantcomplex,C3bBb,iscalledthealternativepathway(AP)C3convertase.C3bBbisdeactivatedinsteps.First,theproteolyticcomponentoftheconvertase,Bb,isremovedbycomplementregulatoryproteinshavingdecay-acceleratingfactor(DAF)activity.Next,C3bisbrokendownprogressivelytofirstiC3b,thenC3c+C3dg,andthenfinallyC3d.FactorIistheproteasethatperformsthesecutswithCR1ascofactor.Clone5G9recognizeseparate,non-overlappingepitopesonC3fragments.LevelsofC3inthebloodmaybemeasuredtosupportorrefuteaparticularmedicaldiagnosis.Forexample,lowC3levelsareassociatedwithsometypesofkidneydiseasesuchaspost-infectiousglomerulonephritisandshuntnephritis.
